Our case managers have a substantial amount of clinical experience to understand not only their own role but the roles of other professional and the health and social field in general.

Our case managers role includes:

We have strong links with other professionals for example - Physiotherapists, Dieticians, Podiatrists. If identified that the individual requires support from another professional, we can arrange this and manage the multi-disciplinary team accordingly.

If required, completing a detailed care plan, setting up a package of care and coordinating/ managing the care package.

We will work in partnership with statutory services to ensure that the individuals needs are met.

Our case managers can refer to appropriate third sector services and charities that are available across Wales and England to assist people with their care and support needs.

Trusted Assessor

A trusted assessor is often someone who can keep an individual safe and independent at home by completing a basic assessment to identify the persons goals and needs. The trusted assessor can then prescribe low level equipment and minor adaptations.

Functional Rehabilitation

We provide rehabilitation programmes tailored to individuals needs and goals.
We can support individuals to improve their independence and confidence with the following:

Should it be identified that an individual would benefit from a multi-disciplinary approach, we have strong links with other professionals such as physiotherapists, and can introduce additional parties and individuals to maximise the rehabilitation process. 

Care and Support

We can assess an individuals care & support needs along with setting up appropriate packages of care for individuals. We can manage the package of care, removing the stress away from the individual themselves and their families. 

Manual Handling

We complete comprehensive manual handling assessments in the individuals own home to look at the environment, carer and carer/family’s needs to ensure that the recommendations we make are practical and effective.

Our process includes:

  • Our OT will complete a home visit to assess the environment and the individuals/carers needs. 
  • We can assist with issuing all recommended manual handling equipment such as mobile hoists, ceiling track hoists, stand aids, slings, standing hoists, walking aids, slide sheets, in-bed management systems.
     
  • Once the equipment is in situ and working seamlessly, our OT will complete a detailed, step by step manual handling plan with training videos for the carers/family to follow and can refer to as and when required. 
  • A comprehensive risk assessment will be written up outlining the risks and recommendations.

  • Our OTs are qualified manual handling trainers so can provide in-depth training to family, carers, professionals and individuals on how to use the manual handling equipment safely. 

We provide advice from the moment you first call us, our team will be able to give expert advice, answer any questions you may have and provide guidance on the next steps. 

Seating & Postural Management

We can provide seating and postural advice to people in their own home or other settings. We will complete a comprehensive assessment, support with trials of equipment and provide recommendation of the most suitable product for purchase.

We can complete an assessment and offer advice and assistance in relation to any equipment that may support you around your home.

We will ensure that the equipment will meet your current and long-term needs before you purchase. We can also source and fit the equipment for you at an additional cost or point you in the right direction of where you can purchase the recommended equipment. 

Housing Adaptations

Our occupational therapists have over 10 years’ experience in housing adaptations, specialising in comprehensive assessments which will help us to identify your needs in relation to any adaptations you require. We can support you to make decisions and choose appropriate solutions to adapt your home to make your life easier, increase your independence and most importantly keep you safe. 

We can assess and make recommendations for the following adaptations:

Hoists

We ensure that the adaptations recommended will meet your current and long-term needs. We take into consideration everything from your medical conditions, the needs of others you live with and your existing property layout amongst numerous other factors. 

We can offer a one-off assessment to provide you with advice to start your journey or we can see you through the whole process from start to finish. If the latter option is preferred, we can project manage the entire project by assisting with planning stages, sourcing suitable items or funding, liaising with the relevant parties to get the job done. We can also liaise with your local housing authority to seek disabled facilities grant funding if eligible to assist you in adapting your home. 

If you would like to fund the adaptations yourselves, one of our unique selling point is that we have expert adaptation tradespeople working within our company. This means we have a one-stop-shop offering and can take you through the entirety of the process from recommendation to implementation. However, there is no obligation for you to use our employees, we will happily work with anyone you of your choosing.

Care & Nursing Homes

We work with Care homes & Nursing Home across England and Wales, providing complex postural management assessments, manual handling assessments and seating assessments. Our Occupational Therapists can provide manual handling training to staff.

We tailor our services to meet the needs of the charity to ensure costs are tightly controlled, maximizing the use of available funds. We complete holistic assessments with recommendations for equipment & major adaptations and complete assessments for more complex cases with people who have ongoing needs.

Housing Associations

The Occupational Therapists within Physical Adaptations have a vast range experience working with housing associations across Wales and South West of England to deliver assessments and reports. Our expertise of the sector makes us an excellent and safe choice to support large development / housing projects or individual resident’s needs.

  • We complete a comprehensive OT assessment in the resident’s home to identify the problems that the person is experiencing and their needs. We will then send a detailed report to the housing association that includes comprehensive and detailed recommendations that will improve the tenant’s independence, well-being and safety.
  • Our services can be tailored to your requirements, we believe in no-one-size fits all packages.

     

  • Extensive experience delivering support services to housing associations during phases of regeneration, renewals or new builds
  • We provide a clear timescale for delivery of assessment and report services with no waiting lists, we aim to see clients within seven days and get the report sent within two days after the home visit. We can respond instantly to urgent requests where we can complete a home visit and send a report on the same day.
  • Not only do we focus on housing adaptations when assessing your residents, we look at the tenant holistically. If required, we will make referrals to relevant services, third sector agencies or charities – for example, if we identity that a tenant needs a referral to the local reablement team then we will complete this referral. 
  • We work closely with staff working within the housing associations such as the housing officers, asset management officers and asset management assistants to ensure that they are aware of our involvement and we can arrange joint visits with these workers if required. We enjoy working collaboratively as it ensures that all involved are kept in the loop so that the tenant receives the highest levels of care across all services.
  • We can source small aids and equipment for your residents for an additional cost or discuss with the tenant about them purchasing the equipment privately – we can assist them with this.

  • Our reports also contein specifications and drawings to ensure accurate communication of knowledge so that installed fixtures meet both present and future needs.
  • We ensure that our recommendations will meet the residents current and long-term needs however we consider all options such as the most cost-effective and realistic option. If we feel that an existing property cannot be adapted, we will recommend re-housing and support with this process.
